Your First Visit to a Doctor

WHAT YOU WILL NEED TO BRING FOR YOUR FIRST VISIT

Crucial pieces of data needed at the first visit:

	Health card
	Referral Forms (if applicable)
	Previous Health Records
	Other pertinent medical records, such as x-rays, EKG's, blood work, lab tests, etc.
	Remember to bring your vaccination cards, especially for children.

Reason for Visit Questionnaire

As for all visits, first or otherwise, you should fill out the questionnaire from the previous page concerning the reason for your visit and bring it with you.
